Job Description:
Part Time, Days. 16 hours/week.Pay range is between
$27.93-$41.89/hr, depending on years of relevant and like

positively contribute to the community.POSITION SUMMARYProvides
clinical intakes for triaging adult behavioral health patients to
appropriate level of care; facilitating referrals to and from the
Outpatient Counseling Center and BCH Inpatient Behavioral Health,
BCH Primary Care Behavioral Health, and external sources. Conducts
assessments and triaging of patients using a biopsychosocial model
informed by a robust knowledge of the Diagnostic and Statistical
Manual of Mental Disorders, Fifth Edition (DSM-5) and use of
evidenced based screening tools. Works collaboratively with a
treatment team, including psychiatrists, psychiatric nurse
practitioners and therapists; providing and promoting the
coordination, collaboration and continuity of care for patients
throughout their treatment course at the Outpatient Counseling
Center. Provides wrap-around services, e.g., referrals to homecare,
prescription refills, family meetings, and wellness checks.
Required to perform clinically related administrative tasks
including documentation in a medical record in a timely, effective,
and efficient manner. Excellent organizational skills and the
ability to work and communicate well with others are critical
aspects of this position.JOB QUALIFICATIONSEducation or Formal
TrainingMaster's Degree in Social Work (MSW) required.Special
Qualifications (licensure, registration, etc.)Licensed Clinical
Social Worker (LCSW) in good standing in the State of Colorado
required.Experience2 years of experience in a psychiatric setting
inpatient and/or outpatient.ESSENTIAL DUTIES AND
RESPONSIBILITIESAdmission Process:Assist in admission and
orientation of patients.Review initial assessment(s) and assist in
completion.Conduct patient assessment evaluation and communication
to the treatment team.Treatment Planning:Provide leadership in
completion of the Master Treatment Plan and ongoing treatment
goals.Develop, coordinate and complete discharge
planning.Facilitate patient access to needed resources and aid in
the development of alternatives as necessary to meet identified
needs.Maintain collateral contacts with family or support systems,
outside agencies and professionals as necessary.Attend and
participate in team clinical supervision, team meeting and
treatment planning.Care Management:Facilitate referrals to and from
Outpatient Behavioral Health Counseling Center, Inpatient
Behavioral Health, Primary Care Behavioral Health, and external
sources and specialty providers.Provide wrap-around services
including, but not limited to, referrals to homecare, prescription
refills, family meetings, and wellness checks.Meet with patients to
assess and evaluate status and discuss measurable treatment
goals.Actively involve the patient, family and support system in
planning and implementation of care.Maintain communication with
patient's referring therapist, psychiatrist and/or primary care
physician.Reports all symptoms related by the patient or observed
to the patient's psychiatrist or psychiatric nurse
practitioner.Discharge:Facilitate discharge planning immediately by
working with the patient's family, significant supporters, outside
providers and community resources.Evaluate the need for post
hospital services and determine the appropriate discharge plan for
all cases.Educate the patient/family as to the options/choices for
the patient upon discharge. Communicate the recommendations of the
treatment team.Complete pre-authorization for post-discharge care
when needed.Collaborate with all providers regarding the discharge
plan, i.e. Turning Point, Recovery Program, Mapleton Counseling
Center, outside therapists, outside psychiatrists, Wardenburg
Clinic, the PCP etc.Whenever possible obtain specific appointments
with all providers following up with the patient after
