As an RN Case Manager with UnitedHealth Group, you'll be responsible and accountable for the coordination of patient services through an interdisciplinary approach, which provides clinical and psychosocial support through the continuum of care.

Positions in this function require nurse licensure and certification based on role and grade level. Licensure includes RN and LPN / LVN, depending on grade level, with current unrestricted licensure in applicable state. These roles identify, coordinate, or provide appropriate levels of care under the direct supervision of an RN Manager / Director. Function is responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting. Function may also be responsible for providing health education, coaching and treatment decision support for patients. Includes Health Coach, Health Educator, and Health Advocate roles that require an RN. *Employees in jobs labeled with 'SCA' must support a government Service Contract Act (SCA) agreement.

Primary Responsibilities:
Continuum of care planning that integrates initial assessment of the patient with appropriate post-hospital discharge services
Must demonstrate excellent communication and collaboration skills
Ability to identify gaps and/or barriers in treatment planning
Provide patient, family, and/or caregiver education
Initiate and ensure completion of all necessary paperwork
Provide referrals for community resources to patients / families / caregivers for ongoing support and treatment
Report all relevant information / orders to providers and care team at patient's next level of care
Must be able to functionally coordinate and discharge plan for all age groups, including the unborn child through geriatric age
Compliance with all regulatory requirements for Joint Commission Standards and CMS guidelines
Work is generally self-directed and not prescribed
Works with more complex issues
Serves as a resource to others
This is a high volume, customer service environment. You'll need to be efficient, productive and thorough dealing with patients, families and caregivers. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

Required Qualifications:
Current, unrestricted RN license in the state of employment
3+ years of experience in a hospital, acute care or direct care setting
Typing skills and have the ability to navigate a Windows based environment
You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
Employees are -required -to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attaine
Full COVID-19 vaccination is an essential requirement of this role. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain -necessary proof of vaccination prior to employment to ensure compliance.

Background in managed care
Case management experience
Certified Case Manager (CCM)
Experience or exposure to discharge planning
Experience in utilization review, concurrent review or risk management

Colorado Residents Only: The hourly range for Colorado residents is $25.63 to $45.72.
