Come be part of an independent, community-focused health system; feel good knowing that your work will make a difference in people's lives and positively contribute to the community.POSITION SUMMARYAs a clinical employee that serves the needs of young, middle and older adults in Behavioral Health Services, the position of licensed therapistcare coordinator is responsible for providing psychotherapy and care management services. The position requires the ability to manage treatment, and continuity of care from admission through discharge and coordination of insurance benefits. The position necessitates leadership skills with the ability to direct and evaluate all aspects of patient care, and to assure that they are performed in a timely, effective and efficient manner. Working knowledge of managed care, its terminology, structure and process is preferred. Excellent organizational skills and the ability to work and communicate well with others are critical aspects of this position.JOB QUALIFICATIONSEducation or Formal TrainingAcademic background in the area of social work, nursing, counseling, psychology, or related field with a minimum of master's degree as required for licensure eligibility.Special Qualifications (Licensure, Registration, etc.)Must be licensed LPCLCSW or eligible for licensure.Colorado LPC or LCSW Licensure required within one year of hire.Knowledge, Skill and AbilityPatient-care model of time-effective therapy.Ability to assess and evaluate a wide range of psychosocial impairments within a patient population 18 through the continuum of life.Working knowledge of current state and federal laws relating to mental health (27-10 and Standards and Guidelines). Working knowledge of Joint Commission rules and regulations.Working knowledge of MedicaidMedicare regulations.Knowledge of healthcare systems and reimbursement systems.Ability to perform thorough social and mental assessments.Skill in the coordination and attainment of healthcare services.Excellent verbal and written communications puter literacy and ability to use word processing.Demonstrated knowledge of DSM-V and its use.Demonstrated ability to deal tactfully with all clients, third-party payors, attorneys, all hospital staff and physicians.Knowledge of and experience with addiction, recovery and mental health issues.Familiarity with chemical dependency issues and 12 step programs is required.Experience2-3 years' experience in a psychiatric setting inpatient andor outpatient preferred.Recovery experience preferred.Experience with a multi-disciplinary team approach.ESSENTIAL DUTIES AND RESPONSIBILITIESProfessional Behavior:Conduct all patient-care activities with respect for patient's privacy and right to confidentiality and autonomy.Use discretion in relaying administrative concerns, decisions or discussion with other staff.Provide in-services for staff, when needed, per specialty licensure.Provide coverage, per Program Manager's request, for vacations, leaves of absence, etc.Maintain certification and licensure per state regulations and as required by discipline andor job qualifications.Assume clinical leadership role for program team regarding referrals, orientations, billing and UR concerns, and adherence to appropriate staffing patterns.Oversees quality indicators at the program level and reports data quarterly to Program Manager.Admission Process:Assist in admission and orientation of patients.Review initial assessment(s) and assist in completion.Conduct patient assessment evaluation and communication to the attending M.D. and treatment team.Treatment Planning:Provide leadership in completion of the Master Treatment Plan and ongoing treatment goals.Develop, coordinate and complete discharge planning.Facilitate patient access to needed resources and aid in the development of alternatives as necessary to meet identified needs.Maintain collateral contacts with family or support systems, outside agencies and professionals as necessary.Attend and participate in team clinical supervision, team meeting and treatment planning.Chart Review:Review patient chartsUpdate treatment plansReview any lab or ordersPatient CareReassessment:Meet with patients (weekly) to assess and evaluate status and discuss measurable treatment goals.Actively involve patient and family in the planning and implementation of care as appropriate.Maintain communication with the patient's referring therapist, psychiatrist andor primary care physician.Management of patient milieu.Participate in Program QASQI as assigned by Program Manager.Ability to assess, treat, care for patients age adolescent to pletion of daily charges.Attends mandatory hospital in-service.Reports all symptoms reported by patient, or observed, to Medical Director, attending M.D., and RN providing nursing services for the program.Thoroughly, effectively, and accurately documents all patient clinical information in the medical record.Effectively manages time in relationship to job dutiesresponsibilities.Develops, maintains, and terminates therapeutic relationships with patientsfamilysignificant other(s) while maintaining appropriate boundaries.Utilization Review:Reviews benefit information and incorporates into treatment planning.Provides regular communication to managed careinsurance provider regarding goals, progress in treatment, attendance and DC planVerifies and updates insurance benefit.Refers identified problems to Quality Assurance, Risk Management and Medical Staff QA.Discharge:Facilitate discharge planning immediately by working with the patient's family, significant supporters and community resources. Seek and maintain current knowledge of reimbursement practices, including knowledge of capitated and roll-in contracts and incorporate this into treatment planning and communication with the Utilization Review coordinator.Encourage each patient to fill out a patient satisfaction questionnaire and outcome study.Assist in completion of patient discharge instructions.Dictate patient discharge summary, where required, preferably within 72 hours.
