RN Outpatient Parenteral Antimicrobial Therapy Program
Half Time, 20 hours/week. Pay range is between $29.48-$44.21/hr,
depending on years of relevant and like experience. Boulder

Primary responsibilities will focus on (1) management of outpatient
parenteral antimicrobial therapy program and (2) new and existing
patient triage under the supervision of the infectious disease
physicians and nurse practitioner. Manage the provision of
individual patient infectious disease services and
coordinate/collaborate with the clinical staff to develop and
maintain or revise the patient discharge and home health plan under
the direction of the physician. Coordinate follow-up with home
health and skilled nursing facilities for patients. Monitor the
provision of home health, infusion services, and outpatient
follow-up care; and communicate regularly with the patient, family,
physician and payers as appropriate.# Play a direct role in
education, training, and monitoring of patients with complex
medical problems.# Work under clinical supervision of the
infectious disease physicians and nurse practitioner. Phone triage
nursing includes but is not limited to acting as a liaison between
physicians, nurse practitioner, and patients.# Manages and
documents patient phone calls pertaining to medical conditions and
questions according to department guidelines. Beacon Infectious
Disease Clinic at Boulder Community Health (BCH) organizes care
around patients, while engaging a multidisciplinary Care Team
working proactively to coordinate and track care for each
individual patient. JOB QUALIFICATIONS Education or Formal Training
None Specified Special Qualifications (Licensure, Registration,
etc.) Colorado RN license in good standing. BLS certification
required by the end of conditional period. Knowledge, Skill and
Ability Knowledge of professional nursing theory and scope of
practice to assess patient needs and administer and evaluate
patient supportive care. Knowledge of organizational policies,
procedures and systems to administer patient care. Knowledge of
CLIA, OSHA, Joint Commission regulations, and corporate compliance
program. Knowledge of common safety hazards and precautions to
establish a safe work environment. Knowledge of medical equipment
and instruments to administer patient care. Skill in using the
nursing process to assess, plan, implement and evaluate interaction
with and care for patients ages 15 and up. Skill in maintaining the
principles and applying and modifying methods and techniques of
professional nursing practices to all patients. Skill in
identifying problems and recommending solutions. Skill in
maintaining complete and accurate patient electronic health
records. Understanding and skill in maintaining department quality
improvement. Skill in working effectively with patients and their
representatives, managers, staff, physicians, other hospital
departments, outside agencies, and the public. Ability to
comprehend and follow both oral and written instructions. Ability
to communicate clearly both in verbal and written communication.
Ability to maintain quality control standards. Ability to respond
calmly and effectively in emergency situations. Ability to
interpret, adapt, and apply written guidelines and procedures to
standard work practices. Experience Previous nursing and medical
experience preferred but will consider new graduates. ESSENTIAL
DUTIES AND RESPONSIBILITIES Electronic Medical Record auditing
including, but not limited to, order management, tasking and
messaging, appointment recall, and inbound fax management to ensure
efficiency and patient safety. Telephone triage calls for
triage/referral in consultation with appropriate clinic
provider(s). Retrieves and responds to phone or other messages in a
timely manner. Documents all incoming and outgoing communication.
Serves as a resource to clinic staff regarding medical information,
patient assessment, and nursing management of illness. Occasionally
assist medical assistants with routine patient care and patient
notification as needed. Set-up outpatient infusion and home health
services, and/or other discharge related care as necessary. Works
with physicians, nurses, and patients to assess the urgency of
patient problems.# Coordinate scheduled appointments based on need
in consultation with the appropriate clinic provider(s). About
