Summary:
Boulder Medical Center (BMC) is currently seeking certified Medical
Assistant (MA) candidates to join our team as a Quality Clinical
Coordinator! This position is a job share working full-time 40
hours per week Monday - Friday,

- Day shift schedules are Monday through Friday. There are
occasional voluntary four (4) hour shifts on Saturday mornings from
8 am - 12 pm for several primary clinics (Family Medicine and
Pediatrics).
- Experience working in both primary care and specialty care
clinics. Job Duties:
- Coordinates healthcare services for patients through assessment
of their chronic conditions and/or other health care needs.
- Utilizes data provided by Quality Program Coordinator from
Population Management using both registry functionality within EHR
as well as reports from Clinical Quality Metrics and Payer Reports
in order to identify gaps in care around evidence-based guidelines.
Utilizes this information to address care opportunities and
suggests adjustments in internal workflow to improve patient health
outcomes.
- Comprehends transition of care and medication
reconciliation.
- Assists providers and quality team in meeting utilization goals
by tracking and following up after ER visits, specialty consults,
and diagnostic testing.
- Collaborates with all members of the healthcare team to include
the patient, members of the interdisciplinary care team, the
patient's family, and the physicians in the clinic.
- Supports the Primary Care Physician (PCP) with care huddles,
and monitoring patients between visits.
- Expand BMC involvement in the medical neighborhood; makes
connections with community services and resources, and develops
care compacts with appropriate specialists and health care entities
(hospitals/urgent cares).
- Assists in the development of departmental policies and
procedures, including assessment techniques to evaluate and track
patient outcomes.
- Documents care coordination activities in appropriate module
database (NextGen, etc.) to ensure proper metric and quality
tracking. Experience and Skills
- Must have exceptional interpersonal and communication skills to
support the development of exceptional working relationships with
patients, their dependents and healthcare providers.
- Relies on experience and judgment to plan and accomplish
goals
- Performs a variety of tasks that require a degree of creativity
and latitude
- Must have a proven commitment to customers that continually
focuses on improvement and enhancement to the service delivery
process.
- Must possess analytical and computer skills with proficiency in
computer and Electronic Health Record systems (EHR) such as MS
Office products and NextGen.
- Must be well-versed in all areas of state and federal
regulations regarding HIPAA. Education:
- Completion of a Medical Assistant (MA) education program.
- Active EMT and / or Surgical Tech certification are acceptable
credentials to substitute Medical Assistant (MA) education.
- Completion of a bachelor's degree with an concentration in
science / health sciences are acceptable credentials to substitute
Medical Assistant (MA) education. Applicant(s) must submit
unofficial college transcripts for Medical Assistant (MA) job
placement approval.
- Previous experience in facilitating quality programs in a
healthcare environment, preferred. Previous Experience:
- Previous experience as a Medical Assistant (MA) in a clinic
setting, preferred.
- Must be familiar with standard concepts, practices and
procedure within a particular field. Salary Range:
- $18.36 - 23.50/hour; depending on experience Location:
