Join us and start doingyour life's best work. SM ThePrimary Care Physician Clinicsare guided by the philosophy of the Patient-Centered Medical Home. The medical home organizes care around patients, while engaging a multidisciplinary care team working proactively to coordinate and track care for each individual patient. As a member of the care team, theClinical Care Coordinatorwill support coordinated care, with an emphasis on prevention, acute care, and chronic disease management for patient populations, including those patients who are most vulnerable. This position is(40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 40-hours during our normal business hours of (7:00am - 6:00pm). Our address is located at 4141 Arapahoe Avenue Boulder, CO. Primary Responsibilities: Act as a resource person in collaboration with physician practices to meet organizational quality and efficiency goals Work with population health outreach, patient registries, and electronic health record to support utilization and quality performance Frequent interaction with local specialist offices or other entities to coordinate care and ensure patient follow up Directly engage patients to through mailings, phone calls or other methods ensure screening and other required preventative tests and appointments are completed Support process improvement to enhance physician and staff workflow Collaborate with RN Care Managers to address patient needs, progress and barriers to patient care Run and edit daily and ad hoc reports using EHR and database programs Provide Emergency Room follow up calls per protocol Provide resource referrals for patients requesting assistance with social determinants of health such as food & meals, housing, home safety, vision & hearing loss, health literacy, and transportation Maintain and update shared documents and resources for care team Periodically reach out to community resource contacts to refresh patient materials and services directory Manage preventative care and chronic disease registries per protocol, including assisting with closing gaps in care reports Quality data validation as needed Reinforce care plans under provider or RN care manager's guidance or documented protocols Schedule patients for services necessary to meet disease management guidelines including follow-up from other levels of care Participate in disease management clinics, such as diabetes management. Duties may include: Documentation of patient participation and results in medical record After training, operation of equipment such as retinal camera Tracking and routing from patients' participation Communicate with the patient's provider, therapist, specialists, and other treatment providers to maintain continuity of care, and take immediate action to problem-solve whenever necessary Participate in quality improvement processes Assist in the ongoing evaluation of PCMH and revision of policies and procedures, as well as in the development of new ones as needed Adhere to best practices and evidence-based guidelines to provide patient-centric care Maintain knowledge of financial considerations and limitations of customers' financial circumstances and insurance requirements Acknowledge billing issues with patient and route to appropriate person Perform effectively in stressful and/or emergency situations Function as physician clinic liaison, providing consultation/problem-solving between programs, medical staff, community professionals, and outside agencies Serve on committees as assigned Attend regional education meetings as required for clinic or practice certifications Adhere to standard precautions as appropriate, which may include: The use of protective barriers, as appropriate (e.g., gloves, masks, gowns, pocket masks, and/or safety glasses) Handling and disposing of infectious waste appropriately Hand washing as appropriate Maintains competency testing as required by direct supervisors When applicable, employees must be able to adjust the essential functions they perform appropriately to the age of their patient/customer. Required Qualifications: High School Diploma / GED (or higher) 1+ years of telephonic customer service experience or in-person customer service experience, handling moderate to complex transactions 1+ years of healthcare experience, including familiarity with medical terminology Basic computer experience using a keyboard and mouse and toggling between screens Basic knowledge of Microsoft Excel (create, edit and save spreadsheets Ability to work any of our 40-hours during our normal business hours of (7:00am - 6:00pm). You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. SM Colorado Residents Only: The hourly range for Colorado residents is $15.00 to $25.19 (add additional reference for sales and physicians).
