difference in patients' lives.Overview of Role:*Must have
experience working with the unhoused/homeless population.*The
purpose of this position is to provide a nurse for medical respite
care through All Roads so they can serve as a member of the respite
care program. As a member of the team this nurse will address the
physical healthcare needs for referred patients in Boulder County
delivering off-site based nursing care. Skills should include the
ability to provide independent off-site, specialized nursing care
which leads to connecting patients back to Clinica Family Health
and Wellness as their medical home and increasing the overall
productivity of the clinic. Certain responsibilities of this job
are likely to change as the team and program further develop. This
role will also assist with responsibilities of homeless outreach,
including weekly time at off-site locations for people experiencing
homelessness performing nurse triage, education, care coordination
and navigation, and care under protocol.ESSENTIAL DUTIES AND
RESPONSIBILITIES:Provides nursing care as defined by Clinica
protocols.Promotes awareness of the health center's services and
supports entry into care, including services for substance use
treatment.Provides independent nursing assessments, triage, and
nurse protocols in a medical respite environment where no other
providers are available.Provides nursing support outside the clinic
in designated collaborating organizations (e.g.,TGTHR, All Roads,
Safehouse and other organizations as identified).Assists providers
or medical team with direct patient care and case
coordination.Provides training, education, and mentoring of nurses
in providing primary care throughout the life span.Triages
patients, both by telephone and walk-in, efficiently and thoroughly
assist when able with patient or medical team education and
training.Acts as lead during off-site care situations.Assists with
implementation of an effective Quality Improvement Program as
directed by CD/CMD or Supervisor.Demonstrates knowledge of the
principles of growth and development over the life span.Completes
and passes an annual competency assessment including age-specific
criteria.Assures complete and accurate documentation in the
Electronic Health Record, including partnering with the Revenue
Cycle Director, EVP of BI/IT, and others to capture the required
documentation elements.Assists AND with orientation, education, and
training of new clinical pod staff in clinical support
activities.Manages projects as assigned CD or CMD, or
Supervisor.POSITION QUALIFICATIONS:Education and Experience:RN
licensure from an accredited school required with 2 years'
experience in primary care.Additional community health, care for
individuals experiencing homelessness, and/or psychiatric
experience preferred.Knowledge, Skills and Abilities:Ability to
independently provide nursing services to patients and apply
nursing protocols within scope of RN license consulting with
provider as needed.Capable of seamlessly transitioning from clinic
setting to homeless venues where healthcare and services are
provided.Ability to flourish in a team system.Sensitivity to low
income, ethnic minority community.Bilingual in Spanish,

$64,391 - $89,132 annually. All individual pay rates are calculated
